If a student has a score of 223 on the ELTiS are they able to speak English as well as written in the classroom setting or home? Are they English conversationalist?

This is a fairly high score in ELTiS 1.0, which measures listening and reading. Since language skills typically do not develop in isolation, there is a high likelihood that the student’s speaking skills are also good. Depending on the instructional tradition and practices in the student’s home country, speaking skills may not be highly emphasized, so they may need a “warm-up period” to develop confidence in their speaking skills. In other cultures, the “four skills” are emphasized equally, needing less of an adjustment period. Students seeking exchange experiences are also typically outgoing, which helps.
